relevant medical history and health information;information about your scar history and treatment (where applicable);
treatment records and clinical notes;
appointment history and correspondence.

assess whether treatment is suitable for you;

maintain accurate clinical records;
manage appointments and communicate with you;
meet professional, insurance and legal obligations.
How your information is stored
Client records are managed securely using WriteUpp, my practice management system.
Paper records, where used, are stored securely and treated as confidential.
Sharing your information
Your information will not be shared with anyone else without your consent unless there is a legal, safeguarding or professional obligation to do so.
Your rights
You have the right to request access to the personal information I hold about you, ask for inaccurate information to be corrected and ask questions about how your information is used.
